JOB SUMMARY: The Stroke Program Coordinator is responsible for organizing and administering nursing aspects of the stroke program. Thi! s position guides the acute stroke response team, coordinates the interdisciplinary stroke team, and participates as a member of the Executive Stroke Team. The stroke program coordinator shall work collaboratively with patients and their families, healthcare providers, community groups to facilitate stroke care throughout the continuum of care. The stroke coordinator will coordinate stroke outreach programs including stroke risk factor screening and community education. The stroke coordinator will organize outreach efforts of the stroke service line to critical access hospitals. The stroke program coordinator shall develop and promote a standard of care consistent with decreasing inpatient resource utilization, and contribute to service excellence at the hospital and outpatient level in accordance with the strategic goals established by Mary Lanning Healthcare.

EXPERIENCE:
Required: Previous experience in Emergency Department, Intensive Care and/orProgressive Care nursing.
Required: Documented/demonstrated leadership abilities.

PRIMARY JOB DUTIES:
1. Adheres to Mary Lanning Memorial Hospital âs Mission , Vision and Values.

3. Exemplifies the practice of nursing in accordance with professional values by serving as a role model and champion of nursing.
4. Provides mentoring, supervision, and guidance to all unit managers, charge nurses and unit staff.
5. Plans, develops and implements stroke team goals and strategies to facilitate achievement of organizational strategic plan and goals.
6. Leads and participates in the practice development of positive staff relationships by r! ole modeling and promoting teamwork, mutual respect, and effective communication.
7. Ensures delivery of compassionate, high quality nursing care and nursing supervision as evidenced by achievement of outcome targets related to stroke specific patient satisfaction, core measure clinical outcomes, and staff satisfaction/engagement as well as achievement of stroke team objectives.
8. Responsible to work collaboratively with members of the acute stroke response team staff to ensure effective communication and support of staff and unit operations.
9. Facilitates acute stroke team response and demonstrates strong leadership and communication skills guiding the interdisciplinary team.
10. Establishes an accountability based culture for stroke core measures that facilitates clinical care delivery decision-making and autonomy of the bedside professional nursing staff.
11. Assures availability of appropriate supplies, equipment and clinical resources for stroke p! atients
12. Establishes standards for stroke patient placement and ! stroke patient care assignments
13. Collaborates with hospitals and professional organizations to facilitate the development of the stroke program
14. Assesses stroke community screening and educational needs
15. Assures completion of at least two community educational events per year
16. Consults with members of healthcare organizations and community agencies to meet the needs of the patients, volunteers, and care providers.
17. Assures completion of emergency personnel education annually.
18. Provides expert level understanding of Joint Commission Disease Specific requirements for Primary Stroke Center Certification.
19. Coaches and educates nurses in the stroke unit.
20. Completes annual stroke competencies for stroke nursing unit.
21. Monitors annual stroke education requirements for all member of the stroke team.
22. Develops, implements, and monitors the appropriateness of the stroke nurse orientation.
23. Oversees the devel! opment and delivery of stroke educational programs and competency assessments to ensure that nursing staff are equipped with sufficient knowledge and skills to provide safe, compassionate, respectful, high quality care.
24. Implements performance improvement initiatives to improve stroke care.
25. Develops, maintains, and implements stroke care policies and procedures to conform to current standards of nursing practice, organizational philosophy, and operational policies while maintaining compliance with Joint Commission standards, state and federal laws and regulations.
26. Lead and facilitate nursing and stroke unit knowledge of and compliance with Joint Commission Primary Stroke Center standards.
27. As a member of the Executive Stroke Team takes responsibility for quality improvement program under the auspices of the Organizational Excellence Committee.
28. Coordinates and/or develops ongoing quality improvement activities for stroke.
29. Demonst! rates knowledge and application of key clinical quality indicators and ! proactively monitors and implements processes to achieve and/or exceed established thresholds.
30. Communicates directly with patients, families, nursing staff, medical staff, interdisciplinary team members, and stroke unit leaders to coordinate care, treatment and services, promote coordination of plans of care and ensure the delivery of high quality patient care.
31. Maintains personal professional development through continuing education pertinent to area of practice.
32. Completes 8 hours of stroke education per year.
33. Participates in community health activities as appropriate.
34. Attends and participates in department, educational sessions, Mary Lanning Healthcare quality improvement program, etc.
35. Performs other duties as assigned.
