This employee is a case manager and primary nurse who uses the nursing process in the developm! ent of the plan of care and manages resources to achieve identified outcomes. DUTIES AND RESPONSIBILITIES :
FINANCIAL:
Meets all documentation and productivity standards as identified by the agency.
QUALITY:
Promotes and uses the nursing process in working with clients and families. Develops, implements, evaluates and revises a plan of nursing care.
Consults with and educates the client and family regarding the disease process, self-care techniques, and prevention strategies.
Complies with all federal, state and local laws and regulations as appropriate for the position. Complies with all policies and procedures of the agency, including those specified in the employee handbook.
Evaluates effectiveness and outcomes of client care.
Documents ou! tcomes of care.
SERVICE:
Accepts case mana! gement responsibilities in assessing client needs; planning, coordinating, and organizing care both within and outside the agency; and referring clients to other services as needed.
Communicates appropriately verbally and in writing with clients, staff , health care providers, referral sources, funding sources and other and other community resources.
Initiates appropriate preventative teaching, refers clients to appropriate community resources and is an advocate for families.

GROWTH:

EDUCATION : Graduation from an accredited school of Nursing . A bachelor's degree in nursing is preferred.
EXPERIENCE : One or more years of nursing experience preferred. Experience with nursing of the child and adult preferred. Knowledge of community resources preferred.
TECHNICAL SKILLS : Adapts to changes in procedures and technology. Willingly learns new skills as required for the position
COMMUNICATION : Effective professional communication skills, both written and oral. Ability to read, analyze and interpret appropriate periodicals, regulations and correspondence pertaining to area of expertise. Ability to present information and respond to questions from employees, clients, managers and the general public
REASONING SKILLS : Ability to solve practical problems and deal with a variety of standards effectively. Ability to interpret a variety of instruction! s furnished in written or oral form. Ability to prioritize, check for e! rrors and spot inaccuracies. Ability to predict the logical outcome of various choices and to make good decisions.
PHYSICAL REQTS : Requires full range of body motion, including unrestricted lifting, pushing and pulling capability. Requires corrected vision and hearing within normal ranges. Will frequently sit, stand, walk, drive, crouch, bend, kneel, climb stairs, lift and carry bulky/heavy items and use strength and dexterity to deliver safe and effective home health care on the basis of need and in accordance with agency policies and practices.
ENVIRONMENT : Frequent exposure to extreme conditions (e.g., temperature, fumes, dust, pets, vermin, and other environmental factors in client homes). Potential exposure to communicable diseases and bodily fluids. May work irregular hours and under stressful conditions.
DRIVING/INSURANCE : Must maintain current driverâs license and auto insurance coverage within the limits specifie! d by VNAâs liability insurance carrier and stated in the VNA employee handbook. Must provide proof of license and insurance upon request and have access to reliable transportation. Must maintain an acceptable driving record as set forth in VNAâs policies and procedures.
LICENSURE : Active license to practice as a Registered Nurse in the State of Nebraska or other nurse licensure compact state. Certified in Basic Life Support.
COMPETENCY: Understands legal and ethical obligations for maintaining professional competency. Participates in professional development activities. Integrates standards into practice. Limits practice according to competence.
