HonorHealth is a non-profit, local community healthcare system serving an area of 1.6 million people in the greater Phoenix area. The network encompasses six acute-care hospitals, an extensive medical group, outpatient surgery centers, a cancer care network, clinical research, medical education, a foundation, and community services with approximately 13,100 team members, 3,500 affiliated providers and nearly 700 volunteers. HonorHealth was formed by a merger between Scottsdale Healthcare and John C. Lincoln Health Network. HonorHealth’s mission is to improve the health and well-being of those we serve.
As a community healthcare system, we have a unique responsibility to keep our facilities as safe as possible to protect our patients and team members. This position analyzes medical record content for presence, accuracy, completeness, timeliness, clarity, authentication, and legibility. This position is also responsible for responding to internal and external release of information requests in accordance to existing laws governing confidentiality and the release of medical records.

- Analyzes electronic records to ensure all patient accounts meet State, Federal, and other regulatory and accreditation rules. Performs these duties in accordance to HonorHealth Medical Staff Rules and Regulation requirements regarding record completeness. Performs chart analysis of medical records for documentation deficiencies and completeness.

Collects and verifies receipt of discharged patient records. Prepares records for scanning and scans records into the electronic health record. Reviews electronic images for quality, correct name, correct account, and patient indexing information. Organizes loose documents and prepares documents for scanning, then reviews electronic images and indexes the documents accordingly.

Reviews release of information (ROI) requests for medical records and/or radiology images to determine validity requests, identity of requestors, and appropriateness of releasing the information based on State and Federal privacy regulations. Assists customers with properly completing requests for medical records, fulfilling those requests via mail, compact disc (CD), electronic mail, or other approved means approved by departmental policy.

Actively participates in department activities. Actively participates in safety initiatives and risk mitigating measures where appropriate. Completes all position and department safety related competencies and requirements on a timely basis.
