Available Position: Three Nurse Practitioners/Physician Assistants. Full-time status with benefits.

This is Government Contact position. Contractor's employees are not considered VAMC employees.

The NP or PA providing services under this contract must live in the local commuting area, defined as within 50 miles from the VA.

Work Schedule: Typically Monday through Friday, 8:00 am to 4:30 pm, although there may need some after hours or weekend coverage based on clinic needs defined by the Grand Island VA CBOC.

Contract Period of Performance: From 12/01/2013 to 04/05/2014 and it may be extended until 07/26/2014, upon acceptable performance and all necessary state reviews and approvals.

Statement of Work:
A. Establish Data Base
1. Obtain pertinent medical and psycho-social history in! cluding: patient profile, chief complaint, history of present illness, past medical history, social history and review of body systems.

2. Perform a physical examination as pertinent, modifying the examination for the nature of the complaint or problem, age/sex of patient and physical condition of patient.

3. Determine patient's mental status (i.e., level of consciousness, orientation, mood, etc.).

4. Order and interpret laboratory, x-ray, diagnostic and/or treatment studies appropriate for complaint, age, race, sex and physical condition of the patient.

5. Obtain the following specimens for laboratory evaluation:
a. Blood (venous, arterial, capillary)

b. Urine

c. Stool

d. Vaginal/urethral/rectal secretions

e. Tissue (superficial)

f. Wound/skin cultures

g. Sputum

h. Other specimens as qualified and authorized by Supervising Physician or service chief.

6.! Record and transmit findings from data base:
a. Summarize pertinent aspects of history

b. Summarize pertinent aspects of physical exam

c. Prepare patient visit summaries/progress-notes

d. Write accident/ incident reports

e. Prepare correspondence

f. Complete insurance forms

g. Complete medical evaluation forms

B. Analyze Data Base

1. Differentiate between normal and abnormal (including variations of normal) information obtained from history, physical, and screening examination of the patients.

2. Preliminary interpretation of diagnostic tests with consultation as necessary (such as routine x-rays of chest and extremities, lab studies, EKG, etc.).

3. Develop initial impression as to severity, etiology and immediacy of patient's problem based on analysis of data obtained from history, physical, laboratory and x-ray studies.

4. Establish diagnosis of common problems with! physician consultation and guidance as appropriate.

C. Develo! p/Formulate Health Management Plan - Develop a management plan considering patient's problem, age, sex, race and condition, conferring with the patient, supervising physician, patient's family and other health professionals as appropriate.

D. Implement and Monitor Health Management Plan.

F. Patient counseling.

Other duties as assigned.

Position Requirements

License: Have an independent unrestricted license in a US state, territory or US commonwealth; provide evidence of demonstrated current clinical competence to perform appropriate primary care services. The NP must be an independent licensed practitioner.

Board Certification /Eligibility: Have a professional degree and education applicable to medical staff members, e.g. N.P., ANP, FNP or GNP.

Education : have a professional degree and education applicable to medical staff members, e.g. N.P.( ANP, FNP or GNP).

Life Saving Certif! ications: BLS and ACLS are required. Successful completion of BLS must be done prior to taking the ACLS course. A current BLS card is required and certification must remain current .

Experience: Be computer literate and have moderate typing skills, 25 words per minute, as the VA utilizes computerized patient medical records. Must have experience with the VA's Computerized Patient Record System (CPRS) or a comparable electronic medical record and electronic order entry system.

Continuing Medical Education (CME)/ Certified Education Unit (CEU) Certified Education Unit (CEU) Requirements: All staff providing care to Veterans are expected to maintain all required training consistent with their licensure and with the Contracting agency's medical bylaws. It is highly preferred that contract staff will undergo Veteran-specific training to manage patients with special needs such as those patients with Post-Traumatic Stress Disorder, Traumatic Brain Inju! ry, Military Sexual Trauma, and Suicidal Ideation and other programs th! at may assist with an understanding of the cultural needs and unique population specific issues associated with military service.

Credentialing: Primary care providers must be credentialed and privileged in accordance with policies and procedures set forth in VA Directive 1663 and Veterans Health Administration (VHA) 1100.19. Porviders must also complete the VetPro process prior to working at VHAROS. VetPro takes a minimum of 2 weeks to process. Evidence of completion of required licensure, credentials, required training, current competencies and background investigation must be validated for all contractor personnel referred to perform services under this contract prior to providing direct patient care.

Citizenship Requirement: Must be a US citizen or provide evidence of eligibility to work in the US.

Background Check: Will need to successfully complete a background security investigation before being appointed into this posi! tion and will need to be fingerprinted per Federal requirements.
