POSITION SUMMARY
The registered (licensed) therapist has the direct responsibility for patient care. Obliga! tions include:
developing and effecting highest standards and techniques of patient care; competence in administration of prescribed therapy; instructing the patient, caregiver, home health aide, or nurse in techniques not specifically required to be conducted by the therapist; maintaining liaison with all members of health care team; evaluating the effectiveness of the exercises rendered the patient; documenting patient care accurately and in a timely manner; and completing continuing education and licensure requirements.
I. ESSENTIAL FUNCTIONS
PRACTITIONER FUNCTIONS Assist the physician in the functional evaluation of the patient and in the development of the individual plan of treatment, including both short and long-term goals. Develop and implement the therapy component of the patient care plan. Provide instruction in technical and compensatory strategies that allows individuals to regain skills necessary to complete self-care, home managemen! t, prevocational, and leisure interest in a safe manner while ! facilitating independence. Provide training in Assisted Daily Living (ADLs) to relearn normal movement patterns. Improve manual dexterity for improved independence with self-care and ADLs. Instruct family and caregivers on the total occupational therapy program and the procedures that will be followed for maximum benefit. Observe and report to the responsible physician the patient's progress, reactions to treatments, and any changes in the patient's condition; modify the plan of treatment as needed. Instruct the patient, the caregiver, and other health team members in methods to improve and correct disabilities. Establish and maintain patient rapport through professional conduct and explanation of treatment plan and goals. Communicate regularly with physicians and other health care team members and participate in clinical team conferences. Plan for patient discharge through realistic and attainable long-term goals which may be modified as condition changes. Prepare the pati! ent and the patient's caregiver for care after discharge. Report to the office manager any significant patient, family, or physician complaints or problems. Supervise the COTA as directed by company procedure Provide accurate documentation in a timely manner. PRACTITIONER - HOSPICE FUNCTIONS:
Assess and implement measures to facilitate the optimum psychosocial, physical, and spiritual outcomes for the patient and family. Formulate and implement a plan of care for assigned patients/families and modify the plan as needed with input of the interdisciplinary team and attending physician. Share observations and coordinate patient/family care with the Hospice team. Assess the home environment for safety and assist the caregivers in providing an optimal level of patient comfort and in! dependence. Teach the patient/family applicable home exercise program(s! ) to promote comfort and safe care at home.
MISCELLANEOUS FUNCTIONS :
Maintain confidentiality of records, conditions, and other information relating to patients, employees, and the company Work independently, requiring minimal supervision in performance of duties Demonstrate an awareness of limitations by asking for guidance when needed Maintain an effective working relationship with all disciplines and promote cooperation among all company team members Follow and support company philosophy, policies and procedures Recognize pertinent and/or potential problems and communicate same to appropriate personnel Demonstrate appropriate behavior when dealing with conflict Channel information appropriately Meet company standards and deadlines Maintain acceptable level of attendance (4-6 absences a year, exclusive Family Medical Leave Act) Willing and available to work as requested Exercise good judgment in performance of duties Maintain an acceptable level of pr! oductivity Demonstrate commitment to Corporate Compliance Program, knowledge of consequences of non-compliance, and obligation to report possible non-compliance. Operate a motor vehicle for business related purposes Effectively integrate corporate compliance into planning and daily operations.

Follow policies and procedures related to safety Conduct visual safety inspections on each visit; identify and report unsafe conditions and correct as needed. Immediately report incidents/injuries to supervisor Incorporate safe behaviors into daily work life Demonstrate appropriate body mechanics in all situations Properly use lift equipment as needed or seek the assistance of another person in the absence of lift equipment.
