Summary of Purpose

Responsible for ensuring medical records are easily retrievable and accurate, and that pertinent information is abstracted in the hospital computer system. Responsible for maintaining security of all patient personal health information and ensuring release of information is carried out according to SMCH policies and procedures and HIPAA regulations.

Duties and Responsibilities

Maintains an orderly appearance of the folders and records within the filing system. Evaluates individual component! s from ancillary departments to accurately process as a comple! te visit record. Files "loose" reports in medical records accurately and timely. Accurately archives individual visit records and complete medical records in a timely fashion. Creates folders for new medical records and those requiring an additional volume. Files completed medical records in proper numerical order using the terminal digit filing system. Maintains and ensures confidentiality of patient information according to HIPAA guidelines, and hospital and department policies and procedures. Responds timely to requests for release of information. Reviews records for required content and, if incomplete, directs to proper individual for completion. Processes physician and other in-house staff requests in a timely manner. Maintains accurate daily and/or monthly reports and logs as assigned. Maintains computer equipment and supplies. Complies with hospital computer system policies and procedures. Maintains MPI in Meditech according to department policies and procedures. Uti! lizes ICR module in Meditech according to department policies and procedures. Orders and maintains office supplies according to policies and procedures. Maintains workstation and common work areas. Maintains compliance with HIPAA privacy policies and procedures. Provides advanced notification of absences/changes in work schedule to Director according to department policies. Communicates the mission of the Medical Records Department. Adheres to department and hospital policies regarding time clock use. Verifies and edits personal information such as address, emergency contact, direct deposit, and W4 information online via HR Payroll Connection.

Education and/or Experience

High School diploma or general education degree (GED) required. At least two years hospital Medical Records Department experience preferred. One to two years office experience and/or training; or equivalent combination of education and experi! ence preferred.

Skills/Knowledge

Position requires go! od phone etiquette. Knowledge of medical terminology. Knowledge of State regulations for both acute and skilled nursing facilities. Ability to work with physicians in a collaborative manner. Basic computer knowledge, as well as experience with Word, Excel and Outlook programs. Basic-to-intermediate keyboarding skills. Position requires understanding of general office equipment, such as copy and fax machines.
